Symptoms:

Destroyed case
Intermediate clutch failure
Worn accumulator bores
Low line pressure
1-2 Slip
Delayed Forward
Delayed Reverse

Cause:
The 1-2 accumulator bore becomes worn or scored from repeated oscillation during normal operation.

Correction:

The 1-2 accumulator bore in the GM 300 and 350 cases can become worn or scored from repeated oscillation during normal operation.
